Well……I loved this so much, I had to make my own. I made mine digitally (pdf)…..so I could share it with other family child care providers. I am more than willing to share a copy with you.

Cute idea to increase physical activity indoors which can be hard. Here is a suggestion – hot glue rug gripper pieces from the dollar store on the back of each square. It works great!!!
